LCD L34538 Eligibility Criteria
Part II required for all diagnoses:PPS <70 AND dependence in 2+ ADLs. These apply in addition to diagnosis-specific criteria below.
- FAST ≥7.1 (7A: fewer than 6 words/day)
- Dependent in all ADLs
- PPS <70
- Secondary condition present
Charting Tips for Audit Defense
- Quote specific words patient can still say
- Name every ADL dependency explicitly
- Track weight every visit with percentage calculation
- Document secondary condition with date of occurrence

FAST 7 without the 'A' designation is insufficient
